Office Policies
Telephone Consultations
If you need to talk to the doctor, please make non-emergency calls during regular office hours when your records are available. Our receptionist will take some preliminary information and let you know when to expect a return call. If you call to request a prescription to be refilled, please be sure to call during office hours and be prepared to give the receptionist your pharmacy's telephone number. Our answering service will answer when the office is closed. Non-emergency messages can also be left with this service. All calls will be returned as soon as possible.
After Office Hour Situations
All of our physicians alternate evening and weekend call duty to provide care after office hours. Our answering service will forward messages concerning your needs to the physician on call.
Filing Insurance Claims and Paying Your Bill
We accept assignment from most insurance companies. If you have insurance coverage through a company that we have contracted with, we require a copy of your insurance card, mailing address, and appropriate referral for services to be provided. Failure to provide these items may require you to pay at time of service. Our office will be glad to file your insurance claim for these providers. If you have any of these policies, you will be responsible for any deductible amounts that your policy may have. All co-pay or co-insurance amounts are due at the time of service. For your convenience we accept payment by cash, check/money order, Visa or MasterCard. If you are unable to pay at time of service, prior arrangements must be made with our patient accounts representative.
Please understand your insurance is a contract between you and the insurance provider. Therefore, it is your responsibility to understand your coverage and exceptions of your policy. Due to this relationship, you are ultimately responsible for your bill.
Past Due Accounts
If your account becomes past due and there is not a valid reason for your payment delay, we will take action to recover the amount due within 90 days of the initial billing. If a problem or question arises, please do not ignore the bills which you receive, but discuss the problem with us. In the event your account is placed with an outside collection agency, you will be charged a collection fee (33% of bill) as well as interest on the amount outstanding.
Confidentiality of Your Medical Records
All your medical records and any information you give our staff members will remain confidential. No information about you or your medical history will be released unless we have a written authorization from you to do so.
